What to do before your first appointment:
Since RHS is a specialist doctor’s office, it is very important that you know your benefits coverage for the services we provide before you arrive. You can use our Insurance Benefits Worksheet as well as our Insurance Coverage/ Self Pay Overview to assist you.
If you have medical records from previous treatment which may need to be reviewed by the RHS physician, you must request that those records be forwarded to RHS by submitting a Medical Records Release to your previous physician’s office. Please submit your request as soon as possible. It can sometimes take up to 4 weeks for us to receive records from another physician’s office.
You will receive a user name and password in order for you to be able to access the RHS Patient Portal in an email when you schedule your appointment. You will need to have your patient history completed on the Patient Portal as soon as possible and no later than 48 business hours prior to your appointment in order for the doctor to have time to review it before your visit. You will receive instructions for accessing the portal once you make your initial appointment.
